Monthly Raffle

To enter your name for the raffle, just fill out the following form. The only "cost" involved is that you will be included in our mailing list, but you can opt out of that at any time by following the directions on the mailings.

Be sure to fill out the form completely. Entries with missing information will be deleted without notice, all fields are required. We need your mailing address so that wecanship your prize to you. This information will not be used for any other purpose and will not be traded, sold, or otherwise made available to any other person or organization.

Below the entry form is the prize list. This list will be amended as prizes are donated and awarded.

Only one entry per month is allowed. Five random prizes from the list below will be awarded to five random entries each month.

AOL users and those who use Netscpe 3.0 or older or browsers other than Internet Explorer may not be able to use this form and may EMAIL their entry, but be sure to include all the required informaion.
